reentry.jpgThe Transitional Learning Center (TLC) is a 36-bed, post acute brain injury rehabilitation facility.  TLC was established in 1982 and has operated continuously since that time, providing residential and out-patient services to adults with acquired brain injury.  The program’s focus is community re-integration. The clinical and residential program provides integrated cognitive, behavioral, physical, living skills, and vocational rehabilitation services.  These services are coordinated through internal case managers and are tailored to individual needs. 

Persons with acquired brain injury often experience changes in physical functioning and mobility, ability to carry out activities of daily living, cognitive-linguistic skills, leisure participation, and ability to engage in productive activities.  These changes frequently prevent community integration, limit the ability to engage in meaningful daily activities, and restrict participation in significant life roles.  Rehabilitation is a systematic, evidenced-based attempt to improve functioning in all life domains through remediation, compensation, use of adaptive equipment, and environmental modifications.  The ultimate goal of TLC’s rehabilitation program is to reduce the impact of activity limitations (disabilities) and participation restrictions (handicaps) on the person with brain injury in order to promote the greatest degree of independence and life satisfaction possible.  During the rehabilitation process and beyond, persons with acquired brain injury deserve to be treated with dignity and respect, in a way that honors their age, recognizes their life experience, and considers family and cultural influences.  Throughout TLC’s rehabilitation process, patients are provided with information, compensatory strategies, opportunities for structured rehearsal of skills, and opportunities to make informed choices.  Patients are provided the opportunity to risk with appropriate supervision, supports and strategies in place.  TLC staff endeavors to deliver rehabilitation services compassionately, with creativity and flexibility, and using multiple modalities to meet individual needs.  TLC employs an interdisciplinary team approach as it is the most appropriate vehicle by which to meet the multiple and complex issues of acquired brain injury.
