3367_857_small.jpgThe Transitional Learning Center (TLC) offers survivors of stroke, traumatic or acquired brain injuries an integrated treatment approach to therapy.  An Individualized Program Plan or IPP drives the course of treatment and assures that all therapeutic departments are working together to achieve the best possible outcome for each patient.  Each modality offers individualized and evidence-based interventions/strategies to remediate or compensate for cognitive deficits in concert with the other modalities providing treatment to the patient.  All departments address sequencing, planning, problem-solving, self awareness, attention/concentration, use of external memory aids/environmental modifications and visual-spatial deficits.  Therapies are functional in their approach and integrated into the community as much as possible.  Real life situations are encountered daily as part of the treatment process.

Every patient admitted to TLC is assigned a TLC case manager who serves as the liaison and advocate for the patient with families, referral and funding sources and the treatment team.  Therapies are functional in their approach and integrated into the community as much as possible.  Real life situations are encountered daily as part of the treatment process.
