Employment Application

The Transitional Learning Center (TLC) Employment application is available in Adobe PDF format. You will need Adobe Reader to view this file. To obtain Adobe Reader, please visit http://get.adobe.com/reader/?promoid=BUIGO for a free download.

The Application can be completed in one of several ways:

1. After the PDF file is displayed, simply type directly on the form. Use your mouse to choose the field which you plan to type. You may also use your tab keys to move from one field to the next. Once you've completed the application you may send it electronically back to TLC by simply clicking on the "Email Completed Application Form to HR Galveston" button for our Galveston location or "Email Completed Application Form to HR Lubbock" for our Lubbock location. This will enable you to send the application form via your email client to the appropriate department.
2. If you are unable to use this method, you may also save the file by clicking on the save button and then attaching the document in an email message to hr@tlcrehab.com if you are applying to a Galveston location.  If you are applying to the Lubbock location, send the document to hrlubbock@tlcrehab.org.
3. Alternatively, Follow the same procedure as above, but rather than send the application electronically, print out the completed form and fax or mail the Application to TLC at the address listed below.
4. Or, simply print the form and fill it out or type it and fax or drop it by the address listed on the application
